Case Management Officer_ ECCP (Contract)

Job Description :

 

The ECCP Patient Navigator has a unique role in patient care. The role of the navigator involves building working relationships, coordinate care and provide support for patient/family and the medical team in addition to assisting in program evaluation/quality improvement initiatives.  

 

The Challenges

  • Assist in facilitating patients’ journey and provide strong support to medical teams in their delivery of quality care, including seamless transfer of patient from NUH main campus to NUH Alexandra Campus.
  • Routinely screen for appropriate medical ward patients for transfer to NUH Alexandra Campus on a daily basis (M-F).
  • Discuss with medical team consultant to ensure medical stability for transfer followed by educating patient and family on services at NUH Alexandra Campus.
  • Actively engage teams from both NUH campuses, highlight patient’s needs to teams to expedite care delivery.
  • Assist in ECCP program evaluation for research and quality improvement purposes. Areas include satisfaction surveys, program adherence, patient safety outcome etc.

 

Job Requirements:

 

Qualification:

Degree in science related disciplines or Nursing

 

 

Experience:

 

  • Minimum 1 year of experience in acute hospital setting would be preferred
  • Good organization skill, i.e. able to manage and track a large amount of information such as patient profiles, medical complexities, appointments etc
  • Excellent Interpersonal skills, sensitive to information overload
  • Able to work independently
  • Equipped with basic computer skills in Microsoft Word, Excel, and PowerPoint.
  • Effective working-relationship builder
